Job Description:
A company is looking for a Sr. Case Manager to work with members
requiring intensive or complex care.
Key Responsibilities
Conduct comprehensive assessments to identify members eligible for
case management
Develop and implement individualized care plans in collaboration
with members and their support networks
Monitor care plans, document interventions, and make necessary
adjustments to achieve goals
Required Qualifications, Training, and Education
Bachelor's Degree in a health-related field and 3 years of clinical
experience or equivalent education and experience
Preferred licenses or certifications include RN, LCSW, APSW, CCM,
LPC/LPCC, or RDH
1-3 years of experience in case management, disease management, or
managed care preferred
Knowledge of healthcare advocacy activities and clinical
terminology preferred
Self-motivated with the ability to work independently
